Posted June 2015: Varsity Singers families, if you wish to pay your student’s annual fees on a monthly basis, your first payment is due in July and should be accompanied by a completed Ada Music Boosters Promissory Note. Please click on the link to obtain the form, print it out, complete and have a parent/s or guardian sign it. Thank you!


Posted November 2015: The Ada Music Boosters has been granted sales and use tax exemption status in the State of Ohio. A copy of the tax exemption certificate can be found by clicking on the following link: AMB Tax-exempt Sales and Use Tax Blanket Exemption Certificate signed 070115.


Posted March 2016: Students participating in choir, Varsity Singers, marching band, and winter guard are assessed fees – some at the beginning of the school year (i.e., “school fees”) and others during the school year (through the Ada Music Boosters). There are often questions about what the different fees are used for. A list of the fee breakdown can be found on our Explanation of Fees page.


Posted August 2016: Anyone planning to transport students for school-related functions (including band/choir/winter guard activities) or pull the Music Booster trailer in the 2016-2017 school year are required to complete a volunteer driver release form. The form is available in the Central Office, but can also be downloaded by clicking this link: 2018-19 Volunteer Driver Form.
